Here's the thing. There is no such thing as a planet with no orks. They reproduce by spores that can survive in space, killing them is a temporary solution that makes it worse in the long run but it's the only option as they do nothing with their lives but fight. You can't just blow up the entire imperium because then they're still everyone else's problem.

Spaceships. They get carried around unintentionally by spaceships. Or splayed everywhere by the destruction of an ork ship.

That depends on time to maturity, especially in such an inorganic environment. People can go missing in the depths of an imperial cruiser, a few fungal growths or dormant spores will almost certainly go unnoticed. Not to mention they are usually released in mass quantities during battles, and in the logistical clusterfuck that is any large scale guard department these things are even less likely to be noticed.

I'm not sure what they aren't resistant to. They'll certainly live through any decontamination that can be safely carried out without killing all the humans.
