As a shooter veteran - not many shooters actually have any considerable degree of inaccuracy - chances are it's almost certainly you, not RNG. It's almost all recoil. Typically aim spread/bloom only exists as a mechanism to dissuade poor fire control (guns start perfectly accurate but the bloom or cone of fire spreads with sustained fire) or as a general balancing mechanism (shotguns have a cone of fire to make them inconsistent outside of close range when the target is in the entire cone of fire. Snipers have huge bloom when hip firing to stop perfectly accurate shooting without the need to scope. Assault rifles do it so they aren't basically an automatic sniper in the right hands, throwing off shots at extreme ranges [damage falloff can also accomplish this])
