Far too often they don't. Which sucks. But they always seem to have already had too much of a personal investment in the situation before it even happened. Which led them to assume they could make the right call. So yeah. You're right that they should. But how often it happens is usually an entirely different thing.
(We can only speak from experience, if someone else's experience with moderators is different, that doesn't exactly effect what I've seen, only adds to the bigger picture.)
