//Actually happens more often than you'd think. Even in the original movie, a crew member was killed in a transporter malfunction. McCoy had very good reason for literally hating the transporters, and at the time it was canonical that most starfleet crewmembers had a distrust of the devices. This would largely go away by TNG and VOY, but in ENT and TOS the technology was new and unstable, and would from time to time screw up with usually fatal results.

Danu gets up and goes to the door of her house, knocking firmly against the doorframe as the portal vanishes and the doorway returns to normal.
She covers her ears when the plant starts to screech, before scooping it up and taking it to the sink for some water. Eventually she places the pot on the windowsill where it would get some sunlight.
