Your enemies getting modifiers from your actions make even less sense. Why would your performance impact the performance of your enemies?
But of course it doesn't stop there. In the explanation, somewhere at the end, is an example of using a medkit. In this examle it is described that you work "against" the person who inflicted the wound to whoever you are healing.
How does this work? Is that person constantly shouting at you while you patch your friend up, trying to get you to loose concentration? Especially considering the guy could, at that point, be already gone or dead.
Anyway, those are my opinions on this.
1. I am supposed to roll the dice, like last time, which is why it is round-based.
2. Yes it's stupid and thanks for pointing that out.
3. Fixing that one.
4. When I say "work against", I mean you do back-and-forth with the player to come to a end. Ex:
Player 1: Where is the wound?
Player 2: (quotes post involving a shotgun and a arm)
Player 1: I begin by searching for anesthetic in the kit.
Player 2: The wound should be loosing lots of blood right now.
You get the picture.

Getting something like this right takes some effort, though, so take your time.
